Before you send off your application make sure you have done the following:

Completed your form fully - have you filled in your Personal Public Service/RSI number?

Included your birth certificate if you do not have a free travel pass, or your pass number if you have one.

Photocopied and included utility bills for the allowances for which you are applying.

Sign the form - if you are unable to sign, have it signed by a witness whose address must be included.
