Britain's Pat Smythe, the world's most successful woman showjumper, has died from heart disease aged 67. The winner of four European individual titles, in 1956 Smythe became the first woman to compete in Olympic Games showjumping and the first to win a medal - bronze.

She was also the first female rider to win the Hickstead Derby, on Flanagan, and won grand prix events on her own horses in more countries than any man or woman since.
