Madam, - I think it extraordinarily careless, not to say insulting, that in reporting the death of the distinguished actress Pat Leavy (April 3rd) you should have described her five times as "Pat Leahy". Mrs Leavy's has been a household name here for over 30 years through weekly TV appearances in The Riordans and Fair City. She has been applauded on all the Dublin stages, as well as in leading theatres in Britain, the US, and Australia.

The only comfort to be gained from this unseemly lapse is that Mrs Leavy, famous for her sense of humour, would surely be saying "And they still don't know my name!" - Yours, etc.,

CHRISTOPHER FITZ-SIMON. Richmond Hill, Monkstown, Co Dublin.

Ed.: The error is much regretted. We apologise for any distress caused to Mrs Leavy's family.
