September 21st, 2005: Mr Walsh fell at his home and fractured his hip. He was taken to Our Lady of Lourdes Hospital, Drogheda.

The plan was to take him to theatre that day, but his surgery was postponed "for logistic reasons".

October 4th, 2005: An actively bleeding duodenal ulcer was also diagnosed while Mr Walsh was in the hospital. It was treated by injection.

October 13th, 2005: Having been stable for a number of days, Mr Walsh was transferred to Monaghan General Hospital to recuperate.

4.30 pm: Mr Walsh arrived in the surgical ward of Monaghan hospital.

11.20pm: nursing records indicate Mr Walsh's blood pressure had dropped and his pulse rate had risen and he was noted to be "clammy and sweaty". The surgical senior house officer was called.

October 14th, 2005, 12.40am: Mr Walsh was transferred to the special care unit in Monaghan hospital.

A second on-call surgical doctor, Kiran Jha (surgical registrar), was informed and he came to see the patient.

1.03am: Mr Jha attempted to transfer Mr Walsh to Our Lady of Lourdes Hospital in Drogheda but it would not accept him.

The hospital recommended that he go to the nearest hospital, Cavan.

1.16am: Cavan hospital was telephoned and would not accept him, saying no intensive-care bed was available.

1.30am: Beaumont Hospital was phoned. It had no intensive-care bed available at that time.

The consultant on call in Monaghan, Roy Cazabon, was informed. He travelled in from his home to see the patient.

Over the next few hours, despite receiving blood transfusions, Mr Walsh's condition gradually deteriorated.

7.15am: Mr Walsh was pronounced dead.
