The reigning champions looked well on their way when half back Yvonne Byrne netted a fifth minute penalty.
A swirling Portlaoise wind backed the Tribeswomen in the early play, and they drew level with open play points from forwards Lorna Joyce and Niamh Fahy.
Mayo's Cora Staunton was struggling with her free-taking, and that was to prove worrying as a 22nd minute Fahy goal added to an Annette Clarke point, handed Galway a five-point lead.
The 50th minute brought Mayo's first score from play through Christina Heffernan, and with five frees from Staunton's heavily strapped right leg, the sides were tied 1-6 apiece, at the end of normal time.
Further points from Staunton and 1999 captain Diane O'Hora early in extra-time ensured their October visit to headquarters.
